Support Coordination

Navigating the NDIS can be challenging, but our Support Coordination services make it easier. We help you understand your plan, connect with the right services, and make the most of your funding. Our goal is to empower you to fully engage with the best supports and achieve your desired outcomes.

Psychosocial Recovery Coaching

Our specialised mental health support aims to empower and support people living with psychosocial disability beyond the conventional therapy model. Recovery Coaching is designed to work alongside participants, in the home and community, to ensure they are accessing the support required to meet their goals.

Social Work

Seed Social Workers work alongside participants to overcome the individual and social structural barriers to their success. The face-to-face service is designed to partner with people to identify the underlying causes of challenges being faced. Our Social Workers are equipped to then work in with multiple stakeholder groups to break down the barriers and establish a strategic plan towards success.

Support Work

Our individual support services are all about you. With one-on-one assistance, you can enjoy tailored support in your home or out in the community. We work with you to design support that fits your life and helps you reach your unique goals. Whether you need help with daily living activities, building skills, or accessing community services, we’re here to assist you every step of the way.
We offer tailored support to individuals, ensuring they can access the care and resources they need to thrive. Our services are designed to foster independence, encourage community participation, and help people achieve their personal goals.

Group Activities

Our group activities are all about fostering connections and building friendships. Delivered in a safe and supportive environment, these activities involve two or more participants and can occur at our hub, or in the community. Whether it’s a group class, social outing, or community event, we create opportunities for social interaction, skill development and shared experiences.

Education Support

Our Seed Education Support program is designed to support kids having meaningful and calmer education experiences. The program offers a range of services including;

  • Advocacy and partnership with your school to ensure you have an education support plan that meets your needs, and the school understands your unique circumstances and needs.
  • Flexible and supported learning environment at our Hub for days when you are unable to attend school.
  • Prep Readiness Program – Supported time in a learning environment to get kids ready for the structure and rules of a school giving them a head start to their first years at school.
  • Senior Transition Program – Designed to work alongside senior students to establish career pathways and transition out of high school into further education and/or employment.

Life Skills Program

The Seed Life Skills Program is a structured program designed to support people to build the fundamental life skills for independent living. This is primarily delivered to youth, and young adults as they prepare for adulthood. The curriculum covers topics such as self-care, home care, cooking, and money management. Within this program, we also support people to set up with our social digital systems such as My Gov and Medicare.

Respite Care

Our respite services provide participants and their support people with a much-needed break. We offer both short-term and long-term respite care, giving peace of mind while ensuring participants receive high-quality, compassionate care. This time can be spent at home or exploring new activities in the community, tailored to the participant’s needs.
